PHP Online Referral System for Physicians 'CCMD'

The CCMD.org is an online referral system that allows physicians to refer their patients to providers. To access the functionalities of the site, physicians need to register/log-in into the website. They can manage the providers based on the predefined types. They will also be able to create the referrals by selecting the provider, providing the patient information, providing the other referral information based on the selected types and by uploading the demographic document in PDF format. The e-signature ability has been facilitated through Sertifi API.

PHP and MySQL were the prime technologies we have used to develop the site. This is a simple site with no fussy technicalities or designs. But we have given enough emphasize on coding so that it comes out as an error free website.
